    Diagnostic evaluation fee includes up to five specimens for aerobic culture. Specimens may not be pooled. There will be an additional $8 fee per specimen over five specimens. Antimicrobial susceptibility testing will automatically be set up on significant pathogens. For the antimicrobial susceptibility charge, see antimicrobial susceptibility.
